Specialized Care for Chronic Pain
Welcome to Specialty Chronic Pain Center. We are currently accepting new patients who are seeking dedicated treatment for chronic pain conditions. Because we adhere to strict Florida State guidelines, our intake process is specialized to ensure you meet the requirements for professional pain management.
Step 1: Pre-Screening Evaluation ($100)
To save you time and ensure you qualify for treatment under Florida law, we offer a Pre-Screening Evaluation for only $100. During this process, our clinical team will review your history to see if you meet the state-mandated criteria for chronic pain management.
Step 2: Your In-Person Appointment
In strict accordance with Florida’s pain management regulations, all appointments must be conducted in person. We do not offer virtual or telehealth consultations for pain management evaluations.
What to Bring to Your First Visit: To provide the best care and remain compliant with state guidelines, please bring the following to your appointment:
- Photo ID: A valid government-issued photo ID.
- Medical Records: Any previous records relating to your chronic condition.
- Imaging: Recent MRI, CT Scan, or X-ray reports (highly recommended for pain management).
- Medication List: A complete list of all current medications you are taking.
Chronic Conditions We Accept
We focus on patients suffering from long-term, debilitating conditions including, but not limited to:
- Post-Motor Vehicle Accident (MVA) Pain
- Neuropathy & Diabetic Pain
- Failed Surgical Pain Syndrome
- Sciatica & Severe Back/Neck Pain
- Multiple Sclerosis, Fibromyalgia, & Cancer-related pain
